Consumer Health Data Privacy Policy

This Consumer Health Data Privacy Policy supplements the Sanara Wellness LLC Privacy Policy. It explains how Sanara Wellness LLC (“Sanara Wellness,” “Sanara,” “we,” “us,” or “our”) collects, uses, discloses, and protects consumer health data.
This policy applies where required by consumer health privacy laws, including laws that may apply to residents of Washington and Nevada.
Sanara provides wellness coaching, nutrition education, and lifestyle support. Sanara is not a medical provider and does not diagnose medical conditions, provide medical treatment, or prescribe medication.
Information provided to Sanara may not receive the same protections as medical information maintained by a healthcare provider covered by HIPAA.
1. Consumer Health Data We May Collect
“Consumer health data” generally means personal information that identifies or can reasonably be linked to a person and that relates to or may reveal their past, present, or future physical or mental health.
Depending on what you voluntarily provide, we may collect information concerning:
- Wellness, nutrition, or lifestyle goals
- Weight or body-composition information
- Dietary habits, food preferences, or food sensitivities
- Exercise, sleep, hydration, stress, or energy
- Symptoms or physical concerns
- Menopause, hormonal, digestive, or metabolic concerns
- Health history or medical conditions
- Medications or supplements
- Laboratory or test information you choose to provide
- Information about prior wellness programs
- Coaching progress and communications
- Appointments or requests for wellness-related services
- Other health or wellness information you voluntarily provide
Sanara does not require you to provide more health information than is reasonably necessary for the service or communication you request.
2. Sources of Consumer Health Data
We may collect consumer health data:
- Directly from you through website forms
- During consultations, intake, or coaching
- Through email, text message, telephone, WhatsApp, video meeting, or chat
- From documents or test information you choose to provide
- From an authorized representative acting at your direction
- From a service provider when necessary to provide a service you requested
We do not intentionally use advertising activity or unrelated website activity to infer a specific medical diagnosis or health condition.
3. Why We Collect and Use Consumer Health Data
We may collect and use consumer health data to:
- Respond to your questions
- Determine whether our services may be appropriate for your stated goals
- Provide wellness coaching, nutrition education, and lifestyle support
- Personalize requested coaching or educational services
- Prepare for and conduct consultations and appointments
- Communicate with you about your participation
- Track information you ask us to track as part of a coaching relationship
- Maintain appropriate client and business records
- Protect the security and integrity of our services
- Comply with applicable legal, contractual, insurance, or recordkeeping obligations
Where required by law, we will obtain affirmative consent before collecting or using consumer health data for purposes beyond those necessary to provide the product or service you requested.
4. Consumer Health Data We Share
We may share consumer health data only as reasonably necessary to provide requested services, operate our business, comply with law, or carry out another purpose disclosed to you.
Categories of recipients may include:
- Authorized Sanara coaches, personnel, and contractors
- CRM, form, scheduling, automation, and client-portal providers
- Secure email, storage, video-meeting, and communication providers
- Supplement or wellness platforms you ask us to use
- Technology providers supporting our systems
- Legal, compliance, insurance, and professional advisers
- Government authorities when disclosure is legally required
Service providers may include GoHighLevel or LeadConnector, Google Workspace, WhatsApp, and Fullscript, depending on the services you use.
Where required, service providers process information under contracts and are permitted to use it only for authorized purposes.
Sanara does not sell consumer health data.
5. Advertising and Analytics
Sanara may use Meta, Google, and similar providers for website analytics, advertising measurement, and generic conversion reporting.
Advertising platforms may receive basic identifiers, device information, website activity, and generic events such as PageView, Lead, Schedule, and Purchase.
Sanara does not intentionally provide advertising platforms with:
- Health or wellness form responses
- Symptoms
- Weight information
- Medical history
- Diagnoses
- Medication information
- Laboratory or test results
- Coaching notes
- Specific wellness progress
Sanara does not knowingly permit third parties to collect the health information entered into our forms over time and across unrelated websites or online services.
Analytics and advertising providers may independently collect device and website-usage information through cookies or similar technologies as described in our general Privacy Policy. Visitors may manage optional tracking through our cookie-preference tool.
6. Consent
When you voluntarily provide health or wellness information for a consultation, form, or requested service, Sanara uses that information to respond to or provide the service you requested.
Where applicable law requires separate consent for collecting or sharing consumer health data, Sanara will request that consent before the applicable collection or disclosure.
You may withdraw consent for future collection or sharing by contacting us. Withdrawing consent will not affect processing that lawfully occurred before the withdrawal.
If certain information is necessary to provide a service you requested, withdrawing consent may limit our ability to continue providing that service.
7. Your Consumer Health Data Rights
Depending on applicable law, you may have the right to:
- Confirm whether Sanara is collecting, using, or sharing your consumer health data
- Access the consumer health data Sanara maintains about you
- Request correction of inaccurate consumer health data
- Obtain a list of third parties or affiliates with whom your consumer health data was shared
- Withdraw consent for future collection or sharing
- Request that Sanara stop collecting or sharing your consumer health data
- Request deletion of your consumer health data
- Appeal a refusal to act on a request
- Exercise your rights without unlawful discrimination

We may request information reasonably necessary to verify your identity and protect your information from unauthorized access or deletion.
If we deny your request, you may appeal by emailing info@sanarawellnessco.com with the subject line Privacy Request Appeal and explaining why you believe the decision should be reconsidered.
8. Deletion Requests
When required by applicable law, an authenticated deletion request may require Sanara to:
- Delete covered consumer health data from active records
- Notify applicable processors, contractors, affiliates, or other recipients of the deletion request
- Direct those recipients to delete the covered information
Some information may be retained where permitted or required for legal compliance, security, fraud prevention, dispute resolution, tax or accounting records, or backup-system integrity.
Deletion from archived backup systems may take additional time where permitted by law.
9. Data Retention
Sanara retains consumer health data only for as long as reasonably necessary to provide requested services and meet legitimate business, contractual, legal, accounting, insurance, and security obligations.
Retention periods may vary according to the nature of the relationship and information involved. When information is no longer reasonably necessary, we will securely delete or deidentify it, subject to applicable law and backup-system limitations.
10. Security
Sanara uses reasonable administrative, technical, and physical safeguards intended to protect consumer health data.
Access is limited to personnel and service providers who reasonably need the information for an authorized purpose.
No electronic system or transmission method is completely secure. Please do not use ordinary website forms, email, text messages, WhatsApp, or other general communications for medical emergencies.
11. Policy Changes
We may update this Consumer Health Data Privacy Policy as our practices, services, or legal obligations change.
The updated policy will be posted with a revised effective date. If a material change requires additional notice or consent, we will provide that notice or request consent before applying the change as required by law.
